Patient Information

Over 49,000 visits realized last year in our Dental, Eye, Medical (dispensary, primary & specialty care), and Behavioral Health Clinics. Caridad Center provides services to uninsured children and adults who reside in Palm Beach County, and live at or below the 200% federal poverty level. Our services are free for patients and their families who do not have insurance. Over 28% of Palm Beach County residents do not have medical insurance. Volunteer doctors, dentists, nurses, dentists, dental hygienists, as well as many others contributed over $9.9 million in patient care services. The goal of Caridad is to help the working poor population stay healthy so they can be productive individuals in our community. Last year our dispensary assisted over 1,500 patients with free prescription medication valued at over $2 million.

New Patients

Proof of income, a photo ID, proof that you live in Palm Beach County, and a birth certificate for children 18 yrs. and younger is required. Applicants must complete the registration process before scheduling an appointment at the clinic. Check the Federal Poverty Level Guidelines 2023 to see if you qualify.

Required Documents

All patients must provide these three (3) items:

Proof of Income

Examples:

  • a paycheck stub
  • a letter from your employer (on business letterhead including company name, address, and phone number)
  • unemployment forms
  • a notarized letter from the person who is financially responsible for the household

Identification Card

Examples:

  • driver’s license
  • counselor card
  • passport
  • other recent I.D. with name and photo

Please note: Children 18 and younger must:

  • be accompanied by a parent or legal guardian
  • bring a copy of their birth certificate

Proof of Address

Examples:

  • phone bill, FPL bill
  • car registration or insurance
  • other recent document received in the mail with your name and address

Our medical clinic offers primary and specialty care services to the poor uninsured children and families of Palm Beach County. Our volunteer doctors and staff offer various specialties including Cardiology, Neurology, Nephrology, Orthopedics, Dermatology, Gynecology/Oncology, Podiatry, Nephrology, Pulmonology, ENT, Gastroenterology, Hematology, Psychiatry, Pulmonology, Endocrinology, Rheumatology, Urology, & Breast Surgery. If there is a medical issue that Caridad is unable to treat, we will work diligently to find a hospital and/or medical clinic that will provide that service at a discounted rate or free of charge. Our goal is to have our patients return to good health, and the Caridad Staff/Volunteers work daily for the achievement of those lofty goals.

Eye Clinic

Over 1,100+ patients received general and emergency services this past year.

Our team of Ophthalmologists and medical staff provide various vision care services for both adults and children of Palm Beach County. Caridad is known by patients and staff as a “Place where miracles happen”. There are numerous stories of children and parents who came into Caridad unable to see, and now have the gift of sight. Caridad would be unable to perform these miracles to so many without your generous support.

Dental Clinic

The Caridad Dental Clinic offers pediatric and adult preventive and restorative care. Our volunteer dentists include specialists in Endodontics, Prosthodontics, Pediatric Dentistry, Periodontics, and Oral Surgery. Our goals is to have our patients and their families achieving good oral hygiene, and the Caridad Staff/Volunteers work daily to achieve those goals.
